2. Services Provided
We provide individualized care and support services tailored to each client’s needs. Your services will be outlined in a written Service Agreement or Care Plan, reviewed and agreed upon prior to the start of care.
Please note: Our services are supportive in nature and do not include medical diagnosis, treatment, or advice. Always consult qualified healthcare professionals for medical concerns.
3. Client Responsibilities
By engaging our services, you agree to:
Provide accurate, complete, and up-to-date information about your care needs and health status.
Treat our caregivers and staff with respect and courtesy.
Inform us promptly of any changes to your circumstances or health conditions.
Provide a safe working environment for our staff.
4. Fees & Payments
All fees will be detailed in your Service Agreement.
We accept private pay, long-term care insurance, and VA insurance.
Payment is due per the terms listed on your invoice, typically within 14 days of receipt.
Late payments may result in additional charges or suspension of services.
5. Cancellation Policy
You may cancel services by providing at least 48 hours’ notice.
Failure to provide adequate notice may result in a cancellation fee as outlined in your Service Agreement.
We reserve the right to terminate services if these Terms are not upheld or if payment obligations are not met.
6. Privacy & Confidentiality
We are committed to protecting your privacy and comply with all applicable laws, including the Health Insurance Portability and Accountability Act (HIPAA), when handling Protected Health Information (PHI).
